CHARTIERS VALLEY BOYS LACROSSE ALUMNI GAME LIABILITY WAIVER
ASSUMPTION OF RISK RELEASE OF LIABILITY, AND INDEMNIFICATION AGREEMENT
I, the undersigned participant, hereby acknowledge and agree that participation in the Chartiers Valley Boys Lacrosse Alumni Game (the "Event") involves inherent risks, including but not limited to, serious bodily injury, permanent disability, paralysis, and death. I voluntarily choose to participate in the Event and assume full responsibility for any risks, known or unknown, associated with my participation.
I acknowledge that the facilities, fields, and equipment used during the Event are the property of Chartiers Valley School District ("District") and understand that the District, its administrators, employees, representatives, and agents are not responsible for supervising or ensuring the safety of participants. I further acknowledge that the Chartiers Valley Boys Lacrosse Organization ("Organization"), including its board members, coaches, volunteers, and representatives, is a voluntary organization facilitating this Event and assumes no liability for injuries or damages arising from participation.
In consideration for being allowed to participate in the Event, I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, discharge, and agree not to sue Chartiers Valley School District, Chartiers Valley Boys Lacrosse Organization, and all affiliated individuals and entities (collectively, "Released Parties") from any and all liability, claims, demands, losses, or damages arising out of or related to my participation in the Event, including those caused by the negligence of the Released Parties.
I further agree to indemnify, defend, and hold harmless the Released Parties from any claims, liabilities, damages, costs, or expenses (including reasonable attorney’s fees) arising from my actions or participation in the Event.
I understand and agree that I am participating in this Event at my own risk, and I assume full responsibility for my own health and safety. I certify that I am physically fit, have sufficiently trained for participation, and have not been advised against participation by a qualified medical professional.
I have read this waiver and release, fully understand its terms, and sign it freely and voluntarily without inducement. I acknowledge that by signing this document, I am waiving legal rights, including the right to sue.
